Diflucan Drug Class and Mechanism

Like other imidazole- and triazole-class antifungals, fluconazole inhibits the fungal cytochrome P450 enzyme 14?-demethylase. Mammalian demethylase activity is much less sensitive to fluconazole than fungal demethylase. This inhibition prevents the conversion of lanosterol to ergosterol, an essential component of the fungal cytoplasmic membrane, and subsequent accumulation of 14?-methyl sterols.[2] Fluconazole is primarily fungistatic, however may be fungicidal against certain organisms in a dose-dependent manner. Interestingly, when fluconazole was in development at Pfizer it was decided early in the process to avoid producing any chiral centers in the drug so that subsequent synthesis and purification did not encounter difficulties with enantiomer separation and associated variations in biological effect.

Diflucan Uses

Diflucan works best when the amount of medicine in your body is kept at a constant level. Do this by taking the medication at evenly spaced intervals. Take as directed since different conditions require different methods of use. This drug is usually taken once daily. Another dosing method involves taking the drug for one week per month until treatment is completed. Continue to take Diflucan until the full prescribed amount is finished even if symptoms disappear after a few days. Stopping the medication too early may allow the yeast to continue to grow resulting in a relapse of the infection.

Diflucan Side Effects

Diflucan may cause stomach upset, loss of appetite, altered taste, diarrhea, nausea, headache or dizziness during the first few days as your body adjusts to the medication. Diflucan Pregnancy

Fluconazole was administered orally to pregnant rabbits during organogenesis in two studies, at 5, 10 and 20 mg/kg and at 5, 25, and 75 mg/kg, respectively. Maternal weight gain was impaired at all dose levels, and abortions occurred at 75 mg/kg (approximately 20-60x the recommended human dose); no adverse fetal effects were detected. In several studies in which pregnant rats were treated orally with fluconazole during organogenesis, maternal weight gain was impaired and placental weights were increased at 25 mg/kg.

Diflucan Overdose

There have been reports of overdosage with DIFLUCAN (fluconazole). A 42-year-old patient infected with human immunodeficiency virus developed hallucinations and exhibited paranoid behavior after reportedly ingesting 8200 mg of DIFLUCAN. The patient was admitted to the hospital, and his condition resolved within 48 hours. In the event of overdose, symptomatic treatment (with supportive measures and gastric lavage if clinically indicated) should be instituted. Fluconazole is largely excreted in urine. A three-hour hemodialysis session decreases plasma levels by approximately 50%.
